Supported Living & Community Outreach.

We provide support and outreach services for adults with learning disabilities and associated health needs, we have built up specialist expertise that enables us to provide quality support to adults with various needs.
Our aim is to support, develop and deliver services that are personalised to the people we support.
We currently provide 24hr support to several supported living houses in Amber Valley and Derbyshire Dales in which with a Learning Disability have individual tenancy agreements.
We also provide a community outreach service to adults to support with becoming more confident, participating in community related activities and maintaining family contacts.

Our aim is to enable you to have control of your own life, whilst building important relationships with others.
All our support is person-centred, which aims on you being in control of your own lives.
We would create a support package, where you would have maximum choice, control and power and the flexibility to your changing needs.
We recognise your right to independence, individuality and empowerment.
We would offer you support to enable you to progress in a variety of ways which includes

  • Finances and economic wellbeing, including budgeting and debt management
  • Maintaining accommodation
  • Employment, volunteering, education and training
  • Relationships with family and friends
  • Leisure, cultural and faith activities
  • Developing self-confidence and community involvement
  • Mental and emotional health
  • Physical health, including exercise and healthy eating
  • Domestic and living skills, including cooking, shopping and public transport

Achieving positive outcomes

Positive Horizons empowers people to achieve the best possible outcomes by giving them greater choice and control over how they live their lives. We encourage each individual to reach their potential by delivering specialist person-centred support that enables them to lead a life that is more enriching and fulfilling. We believe that people with learning disabilities and associated health needs have a right to expect better choice, more opportunities and respect from those who support them.


We are committed to effectively protecting all vulnerable individuals supported by our services from any form of abuse and always aim to ensure ‘best practice’ and partnership working in relation to safeguarding.

We strive to maintain a transparent and open culture where everyone feels safe and able to share concerns. Concerns relating to safeguarding are always listened to and taken seriously. We strive also to enable people who deliver a service to develop their understanding of keeping safe. All staff are trained to a level of competence in understanding safeguarding adults.

Working in partnership
We work in partnership with key agencies to ensure best possible provisions and outcomes in relation to supported living and community outreach as well as working in partnership with the Local Authorities to ensure we achieve best practice. Our aim is for every individual we support to have the opportunity to achieve the best possible outcomes and quality of life.

Presently due to Covid we are unable to meet, however all existing members are kept up-to-date via our Facebook group and by post for those who are not on FaceBook. Members can also join our private members group where you can share good news stories and achievements privately between the group.

Hello my name is Honey and I am first and foremost a much-loved family pet but I’m also a very important part of the of the Positive Horizons family too.

My role is to provide therapeutic enrichment to those who want or need it, I do this by visiting my friends regularly, organised walks, and just generally rolling over for my tummy rub, and bringing a smile to people’s faces.
